· 60/40 % Split for 1.5 years to start you off (yes, you get 60% from all patients you treat). We both work. I will try to pass many of my own and new patients to towards you. · Potential to OWN the whole practise in the 6th year with all my patients built over 28 years-plus, with excellent word-of-mouth referrals, and doc referrals in Northern Ontario! You will transition gently and comfortably, build your own patient load which you will inherit, and inherit all my patients and the full practise later (of course, we can negotiate all equipment and chattels at the time). We have an excellent 4-corner location in the Medical Arts Centre with tons of parking, many family physicians in the building, with great traffic and huge signage facing the road for Medical Centre Foot Clinic and Orthotics!

· I have a home-rental location in the South End, close to the clinic, if you wish to rent, and if it isn’t rented or sold by then

What you need:

• Current Professional Liability Insurance per College standards

• Compassionate and ethical focus on patient care. Most of our patients are repeat patients

• Must maintain and upgrade knowledge and skills with regular Con-Eds

• Must have excellent verbal and written communication skills

• Ability to work independently/team setting

• Current CPR/First Aid certification required

* Must be in good standing with the College of Chiropodists of Ontario.

* Provide treatment/management plans for various foot conditions: corns, calluses, ingrown toenails, nail surgeries, fungal infections, diabetic foot care, wound management, be able to inject local anesthetics and cortisone, provide oral and topical Rx’s, compression socks cert. an asset, ESWT, Laser (we will guide/teach all that you need to learn, if necessary), be able to practise full scope per COCO. Perform detailed biomechanical assessments, prescribe & modify/adjust custom foot orthoses when necessary

Full Time Chiropodist

DESCRIPTION

As a key member of West Champlain Family Health Team (WCFHT), you will assist in the primary care functions and participate in the development, implementation, monitoring and evaluation of chiropody services of the WCFHT. This is a shared position between WCFHT and the Centretown Community Health Centre (CCHC) in Ottawa. The WCFHT component of the position is at our Pembroke location, and provides general chiropody treatment. The CCHC component provides wound care services in Pembroke and Barry’s Bay.

The Champlain Diabetes Chiropody Program (CDCP) through CCHC is an amputation prevention program for adults 18 years and over, living with diabetes, having foot ulcers, open wounds and/or ingrown toenail with bacterial infections in locations all across the Champlain LHIN. The regional program is hosted at Centretown Community Health Centre and chiropodist team members travel to offer clinic days in many locations throughout the region. The program has been made available by the Champlain LHIN to improve access and remove barriers to foot care services for high-risk patients living with diabetes in the Champlain region.

Services Offered by CDCP

• Diabetic foot ulcer treatment and offloading including Total Contact Casting, pneumatic walkers and/or shoe/foot padding.

• Nail surgeries for bacterially infected ingrown toenails putting clients at risk of ulceration.

• Evaluation of current footwear

• Advice on proper footwear.

WCFHT and CDCP have an agreement that 2 days a week (.4FTE) of this position is dedicated to seeing clients of the CDCP. Although the chiropodist’s time is shared between both programs, they remain a full time employee of WCFHT. The CDCP has distinct program criteria, referral processes and electronic medical records system. The work involves operating CDCP clinic days at WCFHT and in Barry’s Bay, replacing other team members at other clinics on occasion, attending CDCP monthly team meetings in Ottawa, and reporting to the CDCP Manager all matters relating to the provision of care of CDCP clients.

REQUIREMENTS

• In collaboration with WCFHT clinical professionals, the chiropodist is expected to implement and discuss appropriate individualized care plans with the client and (where applicable) their caregiver based on best practices

5. Communicating foot health education in a variety of settings to patients, Interprofessional health care professionals (IHPs) or other concerned individuals;

6. Ensuring appropriate antiseptic techniques, safe procedures and isolation practices are maintained in the clinical and/or surgical setting; and providing a safe environment for patients, practitioners and others in the clinic.

Administrative Duties

1. Plans for future clinic needs to ensure that HRFHT clinical goals and objectives are met; and ensure that HRFHT mission and vision statement are at the core of service delivery.

2. Ensures safe working practices and that all clinical equipment is in satisfactory operating condition;

3. Maintains and upgrades knowledge and skills regarding new procedures, methods and equipment through attendance at relevant seminars and educational programs, prepares quarterly reports, and monitors daily statistical data;

4. Prepares work schedules that ensure efficient use of time;

5. Adherence to HRFHT policies and procedures

The successful candidate must be in good standing with COCOO, and have professional liability insurance. New graduates are welcome.

The chiropodist must be willing to practise full scope such as routine foot care, nail and soft tissue procedures, custom orthotic management.

You will be an integral part of the Family Medicine environment and will work closely with doctors, nurses and other healthcare professionals to manage a varied patient caseload in diabetes, musculoskeletal and routine footcare. You will provide assessment and diagnosis, design treatment plans and provide podiatry treatment. You will also educate and advise patients on aspects of foot health relevant to them.
